モデルとして = as a model
～をしていただました = to have had something done for you (as humble as you can be)
"I, Fujimoto Miki, [working] as a model had my makeup done (for me)"
Or some variation of that. My English phrasing there isn't the best.
Ick...obviously I need to work on my Katakana. >.> (or not translate these things first thing in the morning XD)
